2015 OPEN ENROLLMENT INFORMATION

OPEN ENROLLMENT NOV 10- NOV 21, 2014


 

General Information Meetings
All employees, retirees, and spouses are welcome at all presentations.

                             
                                                                                                                                                      
Tuesday, November 4th                  ASC-Basement                                                7:30 am – 8:30 am
                                                            ASC-Basement                                                11:30 am – 12:30 pm
                                                            ASC-Basement                                                3:00 pm – 4:00 pm
 
Times for one on one meetings will be from 7 am- 5 pm at your particular locations.  

 

OPEN Enrollment Process
Enrollment will be done by Colonial through one-on-one benefit counseling sessions.  You will be REQUIRED to have a Benefits Counseling session to enroll in coverage for 2015.  With the Benefits Counselor you will be able to enroll in all benefits you are eligible for, medical, dental, vision, FSA, Cancer, Accident, Short Term Disability, Voluntary life and Critical Illness.

Health Savings Account (HSA) Information

 A Health Savings Account is only available if you elect to have the High Deductible Health Plan (HDHP) for 2014.  The HSA gives you the ability to use pre-taxed deductions to save for future healthcare costs.  If you are enrolling in the HSA for the first time, in order to get the Corporation's contribution or in order to contribute from your pay,  you have to open an HSA at Mainsource Bank on Grant Line Rd.  Amberley Kendall is the contact at that branch. 


For new HSA members and current HSA members if you need to start a contribution or change a current contribution, please fill out the below and return interoffice to:  Deena Burks @ ASC. 

NEW PLAN**Vision Information

Effective 1/1/2015 you will have access to a "true" vision plan.  Below please find the benefit summary on how the plan works.  Anthem will be the carrier for this benefit.  To find in network providers, go to www.anthem.com.
 

NEW PROVIDER**Voluntary Benefits Information

Effective 1/1/2015 we will be moving away from American Fidelity for our voluntary benefits and going to Colonial Life.  They will be offering the same great products that you are used to today but at a cheaper or equivalent cost.  Below are the brochures for the plans that they will be offering.

NEW PROVIDER**403 (b) Information


As a NAFCS employee you have the opportunity to contribute to a 403(b).  Effective 1/1/2015 we will be consolidating our services and using Edward Jones Financial Advisors with One America investment lines.  In the month of November you will have the opportunity to meet with an Edward Jones Financial Advisor to go over your 403(b) options.  Any current 403(b) contributions will stop as of the last pay of 2014.  In order to start or continue any 403(b) contributions,  you must meet with a Financial Advisor.

Vitality

Vitality is the new wellness program offered as to all employees enrolled on the Corporations Medical Insurance.  In order to start getting the benefits of this great program, you first need to complete the online health assessment.  Below are the instructions on how to get started with the program.
